As they say in the CIA manual, "The most efficient accident, in simple
assassination, is a fall of 75 feet or more onto a hard surface...In
chase cases it will usually be necessary to stun or drug the subject
before dropping him. Care is required to insure that no wound or
condition not attributable to the fall is discernible after death."

Someone apparently botched that last part of it in both the Forrestal
and Olson deaths. No autopsy report is included with the official
investigation of Forrestal's death. The official investigation,
itself, was kept secret until it was released last year through the
Freedom of Information Act . When Frank Olson's body was exhumed
through the efforts of his son, Eric, it was obvious that he had been
hit on the head before being "dropped."
